Many serious accidents have occurred because a machine was started when it should have been shut down to prevent maintenance or repair. These incidents are usually the result of stored energy that has not been properly released. Lock-out-tag-out (LOTO) is a method to ensure that machines are shut off until an authorized person releases them, helps to prevent these incidents.

The LOTO system consists of a number of locks, tags, and other devices used to isolate, secure or shut off a device from an electric source. All "affected" and "authorized" personnel must be trained in the use of these lockout/tag-out devices. These devices include, but are not only restricted to chains, locks and padlocks. Each device is singularly identified and labeled, including the date and time of application and the name of the person who is authorized to use it and the reason for the lockout or tag-out (repair or maintenance, for example). It must be stated clearly that these devices can't be removed until an entire LOTO procedure has been completed.
